After four unsuccessful nose surgeries, Ms. N.T.B. had only one wish: to regain a “basic” nose shape like everyone else.

Four times of hope, followed by disappointment…
Sharing about her surgical journey, Ms. N.T.B. (born 196X) said that many years ago she underwent an artificial nose implant and later had a structural rhinoplasty using rib cartilage. However, even after two surgeries, her nasal bridge remained severely deviated. This made her extremely self-conscious whenever someone looked at her directly.
Four years ago, when visiting another well-known cosmetic clinic in Ho Chi Minh City to regain her confidence, she was advised to undergo a structural rhinoplasty using “donated rib cartilage.” According to her, the doctor said, “Donated rib cartilage does not require harvesting the patient’s own rib, so it is less painful and safer.” However, four months later, the tip of her nose began to droop significantly. The doctor then revised it using the same technique, but it became infected, forcing her to remove the implant. Unfortunately, due to improper handling, her nasal tip retracted, the columella collapsed inward, and it almost completely disappeared.
By 2019, despite searching many clinics and hospitals across Vietnam for help, nothing gave her hope. She decided to move to Canada with her daughter to live and work. During this time, because she felt insecure about her appearance, she withdrew from social interactions, avoided even close friends, and her mental health declined. “If it hadn’t been for my husband and daughter constantly encouraging me, I don’t think I would’ve had the strength to get through that period,” she added.
During three years in Canada, she never gave up looking for a miracle. Through friends, she learned about a clinic in Vietnam that had successfully treated many severe failed-nose cases. “I saved so many real-patient videos and watched them over and over every day. So many damaged noses were restored. My daughter and I spent the entire Covid period researching, hoping that one day we could return to Vietnam, meet the doctor, and regain a normal nose shape.”

In March 2022, after an in-person consultation and 3D CT scan analysis, Dr. Nam observed that her columella was completely lost and the tip had severely contracted. He proposed three options:
- Option 1: Use forehead skin to reconstruct the nasal tip.
- Option 2: Use skin from under the chin to support the tip and correct the midline indentation.
- Option 3: Use ultra-firm autologous cartilage to rebuild the columella and nasal tip.
Although the first two options offered high success rates, they would leave significant scarring, so she chose option 3. However, Dr. Nam warned that the success rate was only about 50% because the nose was already deformed, the skin lacked elasticity, and creating a new structural columella would be extremely challenging.

A strict postoperative care process
After surgery, Ms. B. followed an intensive postoperative care plan at AZ NOSE, including daily wound cleaning, removing the splint and stitches, taking medication as prescribed, and receiving plasma therapy to reduce swelling and bruising. Because her case was particularly complex, the doctor monitored her closely for the first 15 days and extended her Nasal tube support (the two small white tubes) from the usual 4 days to 10 days to ensure stronger columella stabilization.
At her one-month follow-up, Dr. Nam confirmed that her nose had improved more than 70 percent, with the nasal tip elongated by over 1 centimeter. To perfect the result, after 1–2 years, when her nasal skin becomes more elastic, AZ NOSE will continue evaluating and planning for a second reconstruction stage.
